Mechanical integrity, on a multi-year clock

A Class II well demonstrates integrity before injection begins and on a repeating interval after that — typically five years. You set the interval in months per well, from that well's own permit, because a permit can require shorter. Five years is long enough that nobody remembers the last one without a record.

Monitoring, every single month

Injection volume and pressure are monitored monthly and reported annually. The gap is not the annual report — it is the month somebody forgot, twenty months ago, that nobody notices until the report is due and the row is blank.

The permitted ceiling, checked every month

Your permit carries a maximum daily injection volume and a maximum surface pressure. Every month you record is measured against them, so an over-injecting well is visible the month it happens rather than the year it is questioned.

Being current on one duty says nothing about the other two.

Rule one

A failed integrity test blocks the well until a passing test is recorded after it

A well that fails mechanical integrity is out of service until it is repaired and retested, or shut in. So a failure puts the well in its own band and keeps it there. Recording another month of injection does not lift it — continuing to inject into a well that failed integrity is the thing the rule prohibits, not evidence that it is fine.

Only a passing mechanical integrity test dated on or after the failure clears it. A passing test from before the failure does not, however recent it looks in the list.

Rule two

A missed month is a gap, not missing data

Every month from first injection to last month is expected to carry a reading. A month with no entry is listed by name as a gap and stays one. Nothing is averaged over it, interpolated across it, or quietly forgiven — a monitoring record with a hole in it is the finding, and you cannot fill it in later from memory.

The month currently running is never counted as missing. Its reading is not due yet, and a tool that nags you for it every day is a tool you stop reading.

Rule three

The ceiling is checked independently of both clocks

A well can be inside its retest interval, carry every month since it went into service, and still be injecting above the volume or pressure its permit allows. That is why the ceiling is a separate axis rather than another date: a well can be perfectly current and still be over.

A well with no permitted figures entered says exactly that, rather than reading as clear. Nothing was checked, so nothing is claimed.

Seven bands, worst first

Every well sits in exactly one band — the worst one that applies — so the register can be read from the top down. A well with nothing recorded is not treated as clean; it lands in the emptiest band of all, because no evidence is worse than bad evidence. An exceedance stays visible on the card even when a worse band won the row.

  • Nothing recorded — no retest interval, no first injection date, or nothing on file.
  • Blocked after failed MIT — failed integrity, no passing test since.
  • MIT overdue — past the retest interval set for this well.
  • Monitoring gap — a month since first injection carries no reading.
  • Over permitted ceiling — a recorded month exceeded volume or pressure.
  • Due soon — retest inside 90 days, or last month's reading outstanding.
  • Current — both clocks good and every recorded month inside the ceiling.

Set per well in months from its own permit. Never assumed, never applied site-wide.

Date, pass or fail, test type, pressure held, who witnessed it and who recorded it.

Volume injected, days injected, average and maximum surface pressure, and who recorded it.
